Whether your child is a starting a new grade or a new school, “back-to-school” time means new shoes, new clothes and new supplies – and a new school physical.
Just moved to the area? A recent school physical and completed form are required for student transfers. Kids planning to participate in fall and winter school sports — football, volleyball, cross-country, swimming/diving, soccer, basketball, golf and cheerleading — also are required to have a current sports physical.

*Pricing valid for students in grades K-12 (through age 18). Not valid for college, university or other postsecondary school physicals.
